Application Process/Guidelines
The Foundation for Financial Literacy is a partner with the Arizona Community Foundation. This partnership allows us to provide an online application for the Grant Application process.

We advise you to read the requirements below to prepare for the information needed to complete the online application.

Application Requirements

A completed cover sheet with the following information:

• Name of the organization
• One paragraph summary of the purpose and activities of the organization or group submitting the funding application
• Executive Director
• Contact person
• Street address
• Mailing address
• Telephone number
• Fax number
• E-mail address
• Project Information
• Total project budget
• Requested amount
• One paragraph summary of the project to be funded


The Proposal

Give a one page description of the project, including activities, structure, personnel, and a timeline.
Answer the following questions using two pages or less:
• How the principles of financial literacy are utilized and promoted in the project.
• What “community” or population will be served and how will community members benefit?
• How many people/children are in the population to be served and what are the ages of the individuals served?
• How will the community/population served be involved with the design and implementation of the project?
• Identify outcomes that can be used to measure the success of the project.
• What evaluation process is used to measure the success of the project?
• What follow-up procedure is used to continually monitor outcomes?
• What qualifications or experience makes your group capable of successfully doing this project?

Indicate tax status 501(c)(3) or grassroots.


Attachments
• Names, addresses, and telephone numbers of the members of the Board of Directors or the group responsible for your organization.
• Names, addresses, and telephone numbers of two business references and two personal references.
• Completed budget sheet.

• List of Advisory Board or governing body for the project
• IRS Determination Letter for 501(c)(3), if appropriate
• Letters of agreement from collaborating agencies/organizations
• Signature of Executive Director or Board Chair/President and date of signature
• Board of Directors list with names, addresses, and telephone numbers
• List of business and personal references with names, addresses and telephone numbers



The Foundation generally does NOT fund:

Loans
Grants to individuals
Scholarships


Please complete all information requested in the Online Application. If you submit an application with any portion of the information that is incomplete, your proposal will not be considered for funding. The Grant Committee’s decision is final and cannot be arbitrated.
